Health insurance companies doing business in Massachusetts are rolling out all kinds of new plans, trying to figure out what kinds of limits and restrictions people are willing to accept in order to save money on their premiums. Tiered plans let customers go anywhere for care, as long as they are willing to pay more for the most expensive doctors and hospitals. New limited networks save more but put the priciest facilities off limits. Can insurers find the right balance between restrictions and savings?
