Tufts Medical Center reaches deal with South End clinic

Boston teaching hospital Tufts Medical Center said Wednesday it has formed a clinical affiliation with South End Community Health Center to improve health care services available to residents of Boston’s South End and lower Roxbury neighborhoods.

The partnership will seek to expand the availability of health care programs in the two communities as well as sending the neighborhood center’s patients to Tufts Medical Center’s campus in nearby Chinatown for more complex care.

The two partners will jointly “address health disparities and culture and language barriers by increasing access to primary care, geriatric services, and women’s health programs” at South End Community Health Center, while also tackling chronic health issues such as obesity and diabetes, they said in a statement.
