A 64-year-old Falmouth man was killed in a head-on crash on Route 28 in Barnstable Monday night, police said.
The man was identified by police as Thomas Levesque, who was operating a Chrysler sedan when the accident occurred at about 8:16 p.m. Monday near Stub Toe Road in Barnstable’s Cotuit neighborhood.
According to Barnstable police, Levesque was driving west when he crossed into the oncoming lane of traffic. He first struck the left side mirror of a Infiniti sedan and then collided head-on with a Mercedes-Benz driven by a man identified by police as Phillip Doherty.
Police said the crash forced the closure of Route 28 in both directions while first responders removed Levesque from the crash scene. He was rushed to Cape Cod Hospital by ambulance where he was later pronounced dead, police said.

Doherty was taken to the same hospital for treatment of his injuries.
The cause of the crash remains under investigation by the Barnstable police crash reconstruction unit.
