Community Health Center gains recognition for care


Lynn Community Health Center recently earned recognition for its work in integrated care, which refers to providing a patient with comprehensive health services that are well-coordinated. The state Department of Public Health presented the center with its Commissioner’s Leadership Award at the department’s annual Ounce of Prevention Conference. The award recognizes an individual, organization, business, program, partnership, coalition, or agency that has shown exceptional leadership in one or more of the conference themes. The Lynn center was honored as a leader in integrated chronic disease prevention. Through the center’s Integrated Care Project, teams of behavioral and medical care providers work together to serve patients who have both physical and psychological needs, with the services provided in a single location.
